If you have ever dealt with a stubborn tendon problem, whether it is tennis elbow, Achilles pain, or a persistently irritated shoulder, you know how frustrating it can be.

You rest it. It starts to feel a little better. You try to return to your favourite sport, exercise routine, work activity, or hobby, and then the familiar ache returns.

The usual explanation is often straightforward:

“You must have overdone it at the gym.”

“It is probably repetitive strain from work.”

“You just need to rest it for a while.”

Physical workload and repetitive movement certainly matter. Tendons respond to the demands we place on them, especially when those demands increase too quickly or exceed the tendon’s current capacity.

However, physical loading does not always tell the whole story.

If you experience one tendon problem after another, or if a single issue refuses to improve despite a well-designed rehabilitation plan, it may be helpful to step back and look at the bigger picture.

This does not mean that tendon problems are inevitable or that you are limited by your genetics.

It simply means that your body may require a more gradual and patient approach to building strength, capacity, and tolerance.

2. Metabolic Health: The Environment Around the Tendon

The way your body processes and stores energy can have a significant effect on tendon health.

When blood sugar, cholesterol, or other metabolic markers remain elevated over time, they may influence the structure and function of tendon tissue.

Persistently elevated blood sugar can cause sugar molecules to attach to collagen fibres through a process known as glycation.

In some people, fluoroquinolones can interfere with the normal maintenance of tendon tissue and increase the risk of tendon pain or injury.
